'It gets worse' would normally start the sentence, then continue "especially 〜".

"I don't like the doctor" sounds like you don't like one specific doctor.
You can say "going to" to refer to the action.

It's a superlative so you should use 'the'.

Use 'hope' when talking about the future.
'Wish' is used for imaginary situations.
